Sizes – This form is 3.5” x 8.5”, with a perforation 1.75” from the bottom to provide a take away Claim Check and a hole near the top of the tag so it can be attached to the article.

Option – this tag can also be produced as a 2 part tag using self-imaging carbonless paper and card. Please inquire.

Colour – Your repair tag will be printed with black ink

Material – 12pt white card

Tag Wires – 12″ wires are available for attaching the tags to articles – $ 8.00 per 100 wires (minimum order)

Production – 4 – 6 business days plus shipping time

Imprint Area – The logo / name imprint area is .75” x 2.75”

Logo – your logo can appear on your repair tag for a one time charge of $20.  A high quality pdf file must be provided.

Numbering – extra charge – $25.00 plus $2.00 per 100
