How to order –
1 – Pick one of the designs or supply your own.
2 – Compose the information you want on your form.
3 – Decide on the quantity
4 – Email to mail@123print.ca or send using the Contact page
We will provide you with a proof for your approval prior to printing.
We offer 4 layouts but you can design your own or fax or scan and email a sample and we will reproduce the form that you are used to. Our prescription forms are available as single sheets, 100 prescriptions to a pad and as duplicate forms made with self-imaging carbonless paper, 50 prescriptions to a pad.
Single – 100 prescriptions/pad
PLAIN PAPER $7.25 / PAD
Minimum 5 pads
Material – 20lb white paper for single forms
Duplicate – 50 prescriptions/pad
2 parts white / yellow self copying $10.95 / PAD
Minimum 5 pads
Size – 4.25” x 5.5”
Material – 20lb white and yellow, self imaging paper for 2 part forms.
Colour – your prescriptions will be printed with black ink.
Production – 3 – 5 business days plus shipping time
We also produce forms in books with a wrap around protective cover. Other features available are consecutive numbering and transfer taping for file attachment.
Numbering – extra charge – $25.00 plus $2.00 per 100
Transfer Taping – extra charge – $25.00 plus $6.00 per 100
